Alpha-2 agonists for ADHD
Alpha-2 agonists bind to alpha-2 adrenoreceptors in the PFC1
They have an effect on
- Inattention
- Hyperactivity
- Impulsiveness
- Tics.3
Alpha-2-agnists are
- Guanfacine
- Clonidine
Both are available as immediate-release or sustained-release preparations
Guanfacine ER and clonidine ER are approved by the FDA for the treatment of ADHD.
The IR formulations of guanfacine and clonidine are sometimes used off-label, especially in patients who cannot swallow tablets or when insurance does not cover the ER form. IR alpha agonists must be taken 3 to 4 times daily, and their use may be limited by daytime sedation.
Alpha agonists may be beneficial in the treatment of children with ADHD and comorbid tic disorders or oppositional behavior.
